The observer and mentee contribute as faculty peers. This form does not assess, rank, or score teaching. It does not connect to Canvas or use AI. Entries remain in the browser unless you choose to save, export, or print. Avoid student identifiers or confidential student work.
Print an empty form for handwritten notes, or complete it first. In the print dialog, select your PDF destination. Enable background graphics for tinted headings; section numbers and headings also distinguish sections without color. Entries are not saved automatically.
